Enrollment/Transcripts

Items needed for Enrollment:

• Proof of Address: A current utility bill - Make sure the service address is visible
• Birth Certificate
• Immunizations or Notarized Immunization Waiver
• Unofficial Transcript from student's previous school
• Withdrawal grades if transferring mid-year
   

To request an official FHS Transcript:

1. Download and complete a Transcript Request & Release Form
          **Please note an ORIGINAL signature is required on the release. Please specify if you will be picking up the transcript, or if it needs to be mailed to a college or university. If it needs to be mailed please include the COMPLETE address of the college or university.

2. Fax or email form to (406) 751-3500 ext. 3467 or mary.gustafson@sd5.k12.mt.us or mail it to FHS Counseling Office, 644 4th Avenue West, Kalispell, MT 59901

3. Allow 48 hours to process the request. In July, please allow one week. 

All employee hiring agencies or verification agencies must fax or email a signed release of information to 406-751-3500 ext. 3467 or mary.gustafson@sd5.k12.mt.us

Flathead High School 2024-25 Registration Handbook

To request a high school in-district transfer, please review the instructions in the High School In District Transfer Request Packet and complete the Transfer Request Form. Per Policy 3142, requests for an in-district transfer must be made by the beginning of the previous semester.  Your guidance office or counselor can help you complete the form. 

Submit the completed form by the posted deadlines to the Superintendent's Office of Kalispell Public Schools, 233 1st Avenue East, Kalispell, MT 59901.  If you have questions, please contact Sherry Hodges-Ward at the Superintendent's Office at 406-751-3400, Ext 3434.
